The 2008
Annual Report has gone paperless and was sent out electronically to all LEA’s on
December 23, 2008. Please check your
electronic mail through the portal access.
The Annual Report and the LHU printout will enable FO’s to complete and
return them electronically. The
inspections certification portion will need to be printed out, signed by the FO
and Chief Financial Officer, notarized and mailed back to the Division. N.J.A.C. 5:71-2.6 (a) requires every Local
Enforcing Agency (LEA) to file an Annual Report by January 31 of each year. In
previous years, you received via U.S. Postal Service a report package
containing a blank Annual Report Form, a Life Hazard Use Printout of your LEA
and a blank LHU Inspections Certification Form.
If you have any questions or require assistance, please reach out to Ms.
Karen Luckie via email Karen.luckie@dca.state.nj.us, include a telephone number where you can be
reached and she will contact you as soon as possible. Results of our beta
testing with working Fire Officials were very positive. They were especially
pleased with the ease and efficiency of electronic reporting. The Division
hopes that you will find this to be the same.
Please note:
Microsoft Excel must be installed on your computer to be able to complete this
report. If you do not have Microsoft Excel please contact Karen Luckie.
